Face Masks And Respiratory Hygiene

The WHO and the US CDC recommend individuals wear non-medical face coverings in public settings where there is an increased risk of transmission and where social distancing measures are difficult to maintain. This recommendation is meant to reduce the spread of the disease by asymptomatic and pre-symptomatic individuals and is complementary to established preventive measures such as social distancing. Face coverings limit the volume and travel distance of expiratory droplets dispersed when talking, breathing, and coughing. A face covering without vents or holes will also filter out particles containing the virus from inhaled and exhaled air, reducing the chances of infection. But, if the mask include an exhalation valve, a wearer that is infected would transmit the virus outwards through it, despite any certification they can have. So the masks with exhalation valve are not for the infected wearers, and are not reliable to stop the pandemic in a large scale. Many countries and local jurisdictions encourage or mandate the use of face masks or cloth face coverings by members of the public to limit the spread of the virus.

Common And Local Adverse Events

Pneumococcal conjugate vaccine

Studies of Pneu-C-13 vaccine indicated that irritability decreased appetite increased or decreased sleep and pain, swelling and redness at the injection site after the toddler dose and in older children, are common side effects. Low grade fever occurred in 20% to 30% or more of vaccine recipients. In adults over 50 years of age, the most commonly reported side effects included pain at the injection site, fatigue, headache and new onset of myalgia, with fever above 38°C occurring in approximately 3% of vaccine recipients.

Pneumococcal polysaccharide vaccine

Reactions to Pneu-P-23 vaccine are usually mild. Soreness, redness and swelling at the injection site occur in 30% to 60% of vaccine recipients and more commonly follow SC administration than IM administration. Occasionally, low grade fever may occur. Re-immunization of healthy adults less than 2 years after the initial dose is associated with increased injection site and systemic reactions. Studies have suggested that re-vaccination after an interval of at least 4 years is not associated with an increased incidence of adverse side effects. However, severe injection site reactions, including reports of injection site cellulitis and peripheral edema in the injected extremity, have been documented rarely with Pneu-P-23 vaccine in post-marketing surveillance, even with the first dose. Multiple re-vaccinations are not recommended refer to Booster doses and re-immunization.

Adults At High Risk Of Ipd

Adults with immunocompromising conditions resulting in high risk of IPD, except HSCT, should receive 1 dose of Pneu-C-13 vaccine followed at least 8 weeks later by 1 dose of Pneu-P-23 vaccine, if not previously received. The dose of Pneu-C-13 vaccine should be administered at least 1 year after any previous dose of Pneu-P-23 vaccine. Refer to Immunocompromised persons for information about immunization of HSCT recipients.

Immunocompetent adults with conditions or lifestyle factors resulting in high risk of IPD should receive 1 dose of Pneu-P-23 vaccine, if not previously received. One dose of Pneu-P-23 vaccine is also recommended for all adults who are residents of long-term care facilities and should be considered for individuals who use illicit drugs.

Some experts also suggest a dose of Pneu-C-13 vaccine, followed by Pneu-P-23 vaccine, for immunocompetent adults with conditions resulting in high risk of IPD as this may theoretically improve antibody response and immunologic memory. However, Pneu-P-23 vaccine is the vaccine of choice for these individuals, and if only one vaccine can be provided, it should be Pneu-P-23 vaccine, because of the greater number of serotypes included in the vaccine.

Adults at highest risk of IPD should also receive 1 booster dose of Pneu-P-23 vaccine refer to Booster doses and re-immunization.

What About The Pneumonia Vaccine

Prevnar 13 is a pneumococcal conjugate vaccine that protects against 13 types of pneumococcal bacteria.

Pneumovax 23 is a pneumococcal polysaccharide vaccine that protects against 23 types of pneumococcal bacteria.

Once vaccinated, most healthy adults develop protection to most or all of these types within two to three weeks.

How Long Does A Pneumonia Shot Last

Types Of Pneumonia Vaccine

The pneumococcal conjugate vaccine also known as Prevenar 13 offers protection against 13 strains of pneumococcal bacteria. This type is given to young children as part of their routine NHS vaccinations. Its also available for adults under 65 through our vaccination service.

The pneumococcal polysaccharide vaccine also known as Pneumovax 23 offers protection against 23 strains of pneumococcal bacteria. This type is given to adults over 65 and anyone with a very high risk of pneumonia.

If I Inadvertently Administer Ppsv23 Less Than 8 Weeks After Pcv13 Do I Need To Repeat The Dose Of Either Vaccine

No, you do not need to repeat any doses. PPSV23 that follows PCV13 at less than 8 weeks may increase risk for localized reaction at the injection site, but remains a valid vaccination and you should not repeat it. The PCV13 dose also remains valid and you should not repeat it either. Never administer PPSV23 and PCV13 during the same visit.

As with any vaccination, there are potential side effects of the pneumonia vaccination. Common side effects include:

Injection site soreness

As with most shots and vaccinations, you may experience pain, swelling, or redness at the injection site .

Less than 1% of people who receive a pneumonia vaccine develop a fever. If your temperature is above 100.4 F , you have a fever.

Irritability

Irritability is a feeling of agitation. When you’re feeling irritable, you’re more likely to become frustrated or upset. In children, this may present as fussiness.

Babies And The Pneumococcal Vaccine

Babies are routinely vaccinated with a type of pneumococcal vaccine known as the pneumococcal conjugate vaccine as part of their childhood vaccination programme.

Babies born on or after 1 January 2020 have 2 injections, which are usually given at:

  • 12 weeks old
  • 1 year old

Babies born before this date will continue to be offered 3 doses, at 8 and 16 weeks and a booster at 1 year.

Indoor Ventilation And Avoiding Crowded Indoor Spaces

The CDC recommends that crowded indoor spaces should be avoided. When indoors, increasing the rate of air change, decreasing recirculation of air and increasing the use of outdoor air can reduce transmission. The WHO recommends ventilation and air filtration in public spaces to help clear out infectious aerosols.

Exhaled respiratory particles can build-up within enclosed spaces with inadequate ventilation. The risk of COVID-19 infection increases especially in spaces where people engage in physical exertion or raise their voice as this increases exhalation of respiratory droplets. Prolonged exposure to these conditions, typically more than 15 minutes, leads to higher risk of infection.

Displacement ventilation with large natural inlets can move stale air directly to the exhaust in laminar flow while significantly reducing the concentration of droplets and particles. Passive ventilation reduces energy consumption and maintenance costs but may lack controllability and heat recovery. Displacement ventilation can also be achieved mechanically with higher energy and maintenance costs. The use of large ducts and openings helps to prevent mixing in closed environments. Recirculation and mixing should be avoided because recirculation prevents dilution of harmful particles and redistributes possibly contaminated air, and mixing increases the concentration and range of infectious particles and keeps larger particles in the air.

How Does It Work

Prevnar 20 is a conjugate vaccine. This means that it contains pieces of sugar-like substances called polysaccharides that typically coat the bacteria but also hide it from our immune system. The vaccine uses only a certain portion of the bacteria not the bacteria itself so its unable to cause an infection.

This conjugate vaccine uses 20 slightly different polysaccharides that are specific to the 20 serotypes and attaches them to proteins that our immune systems can recognize. If the bacteria enters the body after the vaccination is administered, the immune system can recognize the polysaccharide molecule and release antibodies to fight the bacteria before it causes an infection.

How Many Doses Of Pcv13 Can An Adult Get In A Lifetime Who/when

CDC recommends adults receive 1 dose of PCV13, if indicated and if they have not received PCV13 previously . In addition, adults age 65 or older who do not have an immunocompromising condition, cerebrospinal fluid leak, or cochlear implant can choose to receive PCV13 based on shared clinical decision-making. However, if an adult received a dose of PCV13 prior to turning 65 years of age , they should not receive a dose of PCV13 when they turn 65.

Who Should Have The Pneumococcal Vaccine

Anyone can get a pneumococcal infection. But some people are at higher risk of serious illness, so it’s recommended they’re given the pneumococcal vaccination on the NHS.

These include:

  • babies
  • adults aged 65 or over
  • children and adults with certain long-term health conditions, such as a serious heart or kidney condition

Babies are offered 2 doses of pneumococcal vaccine, at 12 weeks and at 1 year of age.

People aged 65 and over only need a single pneumococcal vaccination. This vaccine is not given annually like the flu jab.

If you have a long-term health condition you may only need a single, one-off pneumococcal vaccination, or a vaccination every 5 years, depending on your underlying health problem.

What Are The Possible Side Effects Of Pcv And Ppsv Vaccines

Kids may have redness, tenderness, or swelling where the shot was given. A child also might have a fever after getting the shot. There is a very small chance of an allergic reaction with any vaccine.

The pneumococcal vaccines contain only a small piece of the germ and so cannot cause pneumococcal disease.

Viral And Host Factors

Virus proteins

Multiple viral and host factors affect the pathogenesis of the virus. The S-protein, otherwise known as the spike protein, is the viral component that attaches to the host receptor via the ACE2 receptors. It includes two subunits: S1 and S2. S1 determines the virus-host range and cellular tropism via the receptor-binding domain. S2 mediates the membrane fusion of the virus to its potential cell host via the H1 and HR2, which are heptad repeat regions. Studies have shown that S1 domain induced IgG and IgA antibody levels at a much higher capacity. It is the focus spike proteins expression that are involved in many effective COVID-19 vaccines.

The M protein is the viral protein responsible for the transmembrane transport of nutrients. It is the cause of the bud release and the formation of the viral envelope. The N and E protein are accessory proteins that interfere with the host’s immune response.

Host factors

Human angiotensin converting enzyme 2 is the host factor that SARS-COV2 virus targets causing COVID-19. Theoretically, the usage of angiotensin receptor blockers and ACE inhibitors upregulating ACE2 expression might increase morbidity with COVID-19, though animal data suggest some potential protective effect of ARB however no clinical studies have proven susceptibility or outcomes. Until further data is available, guidelines and recommendations for hypertensive patients remain.

Concurrent Administration Of Vaccines

Pneumococcal vaccines may be administered concomitantly with other vaccines, with the exception of a different formulation of pneumococcal vaccine . There should be at least an 8 week interval between a dose of pneumococcal conjugate vaccine and a subsequent dose of Pneu-P-23 vaccine, and at least a 1 year interval between a dose of Pneu-P-23 vaccine and a subsequent dose of pneumococcal conjugate vaccine refer to Immunocompromised persons for information regarding administration of pneumococcal vaccines to HSCT recipients. Different injection sites and separate needles and syringes must be used for concurrent parenteral injections. Refer to Timing of Vaccine Administration in Part 1 for additional information about concurrent administration of vaccines.

The pneumonia vaccine is a safe way to prevent most cases, and effectively lowers the chances of catching the disease entirely. People who do get pneumonia after getting the pneumonia vaccination anyway will have a much milder case of the disease if they do happen to contract it.

Are There Treatments For Covid

Who Should Not Get The Vaccine

People should not get the vaccine if they have had a life threatening allergic reaction to a previous dose.

Additionally, a person should not undergo vaccination if they have had an allergic reaction to medication containing diphtheria toxoid or an earlier form of the pneumonia vaccination .

Lastly, people who are sick or have allergic reactions to any of the ingredients of the vaccine should talk to a doctor before getting the shot.

A pneumonia shot will not reduce pneumonia. However, it helps prevent invasive pneumococcal diseases, such as meningitis, endocarditis, empyema, and bacteremia, which is when bacteria enter the bloodstream.

Noninvasive pneumococcal disease includes sinusitis.

There are two types of pneumonia shots available. Which type a person gets depends on their age, whether or not they smoke, and the presence of any underlying medical conditions.

The two types are:

  • Pneumococcal conjugate vaccine : Healthcare providers recommend this vaccine for young children, people with certain underlying conditions, and some people over the age of 65 years.
  • Pneumococcal polysaccharide vaccine : Healthcare providers recommend this vaccine for anyone over 65 years of age, people with certain underlying conditions, and people who smoke.

  • roughly 8 in 10 babies from invasive pneumococcal disease
  • 45 in 100 adults 65 years or older against pneumococcal pneumonia
